Safety Agreement

HEALTH, SAFETY AND SOCIAL DISNACING PROTOCAL.

Please review our health, safety, and social distancing protocol with your student. Anyone in violation of these protocols will be removed from the service. Each section requires you to check the box stating that you agree and will comply by these protocols while on campus at the service.

SOCIAL DISTNACING

All Students are expected to follow social distancing guidelines of keeping 6ft apart from each other as best as possible. All chairs and activities will be adjusted to being spaced out and socially distant. Failure to comply to the guidelines will result in the student being excused from the service.

FACIAL COVERING

Masks are required on campus and if you don’t have one, we’ll provide one for you! Face coverings will not be required while a student is eating but is required during all other times. Masks must fully cover the mouth and nose area.

NO PHYSICA CONTACT OF ANY KIND

Again, we reiterate for the service to be safe and successful, all students must comply with keeping 6ft apart. No hugging, high fives, fist bumps, and any other form of physical contact are to take place on the church premises.

TEMPREATURE AND HEALTH CHECK

Upon arrival to the campus, all students will be required to check-in and have their temperature taken with a no-touch thermometer.

*If the student’s temperature is at 100 degree or greater, we will have to ask that your student excuses themselves from the service.

SANTA CLARA COUNTY COVID-19 HEALTH ORDER COMPLIANCE

I will make sure on the day of the service that I can honestly answer:

-My student/any member of our household has NOT had any signs or symptoms of a fever, worsening cough/shortness of breath or lack of taste/smell in the past 24 hours.

-In the last 14 days, my student/any member of our household has NOT been in close contact with someone with similar symptoms or a diagnosis of COVID-19.

-I have Not traveled outside of the country in the last 14 days

*And if on the day of the service I cannot honestly answer these statements, then I will excuse myself from attending the service.
